The ICD coding manual is the reference book used to assign diagnosis and procedure codes to medical documentation that is abstracted from the patients medical chart. It contains different volumes which categorize codes. Symbols are used to give definition of meaning to each code description. The manual serves as the guidebook for code assignment.
